Report on the accident involving 1007 "Western Talisman" on a Paddington to Oxford service. A locomotive battery box door was insecurely fastened, fouled the points and caused the derailment in which ten passengers died.

Fatal collision of Peak Class 45 No. 125 on a Derby to Exeter freight with the rear of another freight hauled by Class 47 47441. Although extensively damaged the Peak was repaired and renumbered as 45071, the last locomotive to receive its TOPS number.
