Derailment of Britannia Pacific 70026 "Polar Star" hauling an excursion train from Treherbert to Paddington. Caused by excessive speed through a crossover, the engine and three coaches rolled down an embankment with 11 fatalities.

Handrails on the Britannia's smoke deflectors was considered to have contributed to the accident by obscuring vision and the class was modified to have hand holds rather than rails.
